Submit the following as part of your application:

• Completed Wake Forest supplement ONLY if you are using the Common Application or the Universal College Application
• $50 application fee

• Schedule an on-campus interview (strongly recommended); interviews are available via Skype or online for students unable to travel to campus. Contact the Admissions office at 336.758.5201 to schedule your interview. Find more details here.
• Provide the Secondary School Report form [PDF] to your principal/college advisor/guidance counselor for completion
• Provide the Teacher Recommendation form [PDF] to a teacher in an academic subject for completion; include a stamped envelope addressed to WFU Admissions
• Provide the Mid-Year School Report form [PDF] to your principal/college advisor/guidance counselor for completion
